WAIVER AND RELEASE OF LIABILITY

Effective Date: March 30, 2021

PARTICIPATION DISCLAIMER:

In consideration of the risk of injury that exists while participating in a workshop (hereinafter referred to as “Activity”) and my desire to participate in said Activity, I agree to the following terms:

TERMS AND CONDITIONS:

Acknowledgment of Risks:
- I am voluntarily participating in this Activity at my own risk.
- I acknowledge the risks associated with this Activity, including but not limited to:
  - Physical or psychological injury, pain, suffering, illness, disfigurement, temporary or permanent disability (including paralysis), emotional loss, and death.
Waiver of Claims:
- I waive all rights, claims, or causes of action arising out of my participation.
- I release and discharge Saint Lucie County Rock and Gem Inc., its affiliates, and all associated parties from any liability for injuries or damages resulting from my participation.
Medical Authorization:
- In the event of medical treatment, I authorize emergency care as deemed necessary, including CPR, AED use, and transport.
- I accept financial responsibility for any incurred costs.
Indemnification:
- I agree to defend and hold harmless the Releasees from claims, damages, or costs related to my participation in this Activity.
Activity Requirements:
- I will not participate unless I am medically able and properly trained.
- I will abide by decisions of authorized officials regarding my participation.

LEGAL ACKNOWLEDGEMENTS:

Understanding of Terms:
I have carefully read and understand this waiver and release of liability.
Legal Agreement:
This release is governed by Florida law and is a binding agreement between equal parties.
Liability for Damages:
I accept liability for damages caused by my willful actions or neglect.
